The CVS Health Foundation is planning to launch the Focus on Healthy Aging: Multi-Year Funding for Systems of Care for Older Adults Grant in October 2024. This new, five-year initiative will provide funding for organizations that work to implement strategies that improve health outcomes for adults 60 years or older. The initiative will evaluate applications by factors including geography, social isolation, race/ethnicity, limited English proficiency, and socioeconomic status to determine communities with the highest risk of negative brain, heart, and/or mental health issues.
